Feedback and complaint reporting

EQUS is committed to fostering an equitable and inclusive culture that promotes a sense of belonging among our members, where all individuals feel valued and respected.

If an EQUS member finds themselves in a situation where they do not feel this way, because of the actions or behaviour of another EQUS member and/or group, or feels unreasonably disadvantaged or discriminated against because of an EQUS procedure or process, we want to know.

This form enables EQUS members to provide feedback or report incidents in a way that provides maximum safety and comfort.

All information provided will be handled confidentially.  Submission details will be sent to the EQUS Chief Operating Officer (Ilan Mears) only, unless an alternative contact person is selected in the form.  In this case, details will be sent to that person (and only that person).

Feedback and reports may be provided anonymously; however, options for resolving anonymous reports may be limited.

For non-anonymous reports, we will get your consent before sharing your information with other people or organisations, except in circumstances where we are obliged by duty of care to disclose certain information or report notifiable incidents.  After submitting this form, subsequent discussions will be arranged only at your request (if indicated in the form).

In any circumstance, EQUS members may contact the Centre Director, Chief Operating Officer, Deputy Directors or relevant Node Director directly, and do not need to complete this form to do so.
